Remote Patient Monitoring (RPM)
HeartCloud's platform includes everything a medical practice needs to start, operate, and generate consistent revenue each month from insurance reimbursements for remotely monitoring blood pressure, weight, and other cardiovascular and neurological data points collected by devices deeply integrated with our patient and provider apps.
Overview:
Outside of a doctor's office, patient physiologic measurements are a better reflection of reality. To help get this data to a clinician, and to help patients take readings on at least 16 out of 30 days for billing purposes, our platform focuses on reliable, automatic uploading.
Turn Digital Health Data Into Monthly Revenue:
Our platform automates generating claims for CPT codes 99453, 99454, 99457, 99458, 95249, 95250, 95251, and many more. It tracks readings from devices against policies specific to a patient's insurance coverage, ensuring compliance and maximizing the probability to obtain reimbursement payments each billing cycle.
Built By Practicing Physicians For Their Own Patients:

Our platform automatically consolidates measurements from multiple devices to give clinicians an integrated, longitudinal view of patient health trends.

For all patients or on a per-patient basis, notification thresholds can also be sent during scheduled hours to one or more phone numbers. This flexibility and timeliness lets clinicians respond more proactively to real-world patient data.

A built-in ordering system lets your practice ship devices pre-configured with a patient's ID directly to their home (or non-configured devices to hand out in person).

Each patient gets access to a web-based patient portal on https://heartcloud.io and an iPhone-based patient portal app named HeartCloud Sync.
Patients enrolled in our tremor monitoring product using Apple Watch can use their device, at no extra capital expense to the practice, with HeartCloud using a dedicated Apple Watch app.
