Orthostatic Hypotension:

Our patient-facing app, HeartCloud Sync, includes a guided orthostatic blood pressure test. The user is guided by a timer and easy-to-follow instructions to take blood pressure readings while both sitting and then standing. Captured measurements and which reading was taken in what postural state are automatically uploaded and made available in the patient's EMR chart.

Every day at the same time (or on a schedule set by the patient's clinician), a text message is sent to a patient with a survey that takes less than 15 seconds to complete
Designed by neurologists who use it with their own patients, these daily questionnaires collect pertinent data every day regarding the presence or absence of migraines on a given day.
If a migraine is present, it asks the patient to detail common daily life activities that the migraine interfered with, whether or not rescue medications were taken, and the presence or absence of brain fog.

How Do Patients Sign Up?

Through a QR code. Our team generates a tri-fold brochure customized to your practice name, logo, and other brand elements. This print material is available to patients in your clinic office.
Patients use their smartphone camera to scan a QR code on the brochure (which is unique to your practice). This opens a link on HeartCloud's website, where the patient enters his or her cell phone number. The cell phone number is checked against your EMR records (both so that a non-patient cannot sign up and to verify clinical and insurance eligibility).
